There are two methods to complete the forms.  There are forms that you can complete on line (using Microsoft Word) that can be printed and sent back to the Field Office.  These forms are listed on the bottom of this webpage.

The second method (of the same form) is to print the form out using Adobe Acrobat from the list immediately below.  You will need Adobe Acrobat to use these forms.
